A dilemma in orthodontics: Extractions in borderline cases

چکیده
Patient with good facial esthetics require extractions to reach a stable and functional occlusion can be categorized as a borderline case. It may also be defi ned as the case caught in between the confl ict of extraction and nonextraction. Empirical evidence of uncertainty exists with these patients. Borderline cases may also have an absence of dental or craniofacial anomalies, permanent dentition, healthy periodontium and normal anteroposteriorly relationship between maxilla and mandible. Therefore, the aim of this paper is to describe criteria, which should be kept in mind before deciding to go for extraction or nonextraction treatment in borderline cases.

One of the reasons for this change in approach is based on the findings of studies published in the early 1980s by researchers at the University of Washington, who reported similar rates of teeth alignment relapse in cases treated...

In planning orthodontic cases that include extractions as an alternative to solve the problem of negative space discrepancy, the critical decision is to determine which teeth will be extracted. Several aspects must be considered, such as periodontal health, orthodontic mechanics, functional and esthetic alterations, and treatment stability. Despite controversies, extraction of teeth to solve de...
متن کاملExtraction decision and identification of treatment predictors in Class I malocclusions
BACKGROUND The extraction rate in orthodontics varies throughout the years. While the extraction decision is easily made or excluded in clear-cut cases, it still remains controversial what makes an orthodontist decide to extract in borderline cases. The aim of this retrospective study was to identify the percentage of extraction cases in a large group of Class I malocclusions and to clarify whi...
متن کاملAtypical Therapeutic Extraction in Orthodontics (Mandibular Incisor Extraction): A case report
Extraction versus non-extraction debate has been a major controversy in orthodontics and it is still continuing since Angle era. Extraction of premolar teeth is the conventional treatment of choice for the management of class I malocclusion cases but in border line cases, orthodontists find difficulty in making a decision particularly in cases with good facial esthetics. However class I maloccl...
متن کاملEvaluation of the role of unilateral and bilateral tooth extraction treatment on maxillary arch symmetry in Class II subdivision cases
Background and Aim: Unilateral extractions have been proposed as potential treatment options to correct Class II subdivision malocclusion, suggesting that most side effects associated with asymmetrical mechanics can be avoided, however the post-treatment arch form may not be symmetrical. Therefore, the aim of this study was to determine the post-treatment effects of unilateral premolar extrac...
